Electronic Disclosures Consent

As part of the Home Equity Loan application process, federal law requires that we provide important disclosures to you. For your convenience, you may choose to receive these disclosures electronically. You can print and download the disclosures we provide electronically for your future reference.

If you would like to receive your disclosures electronically, you must carefully review the following terms and affirmatively indicate your consent by checking the "I Agree" box below.

1. Your consent to receive electronic disclosures only applies to this loan application. All other disclosures and notices we provide to you in connection with the loan process will be in writing unless you and we agree otherwise.

2. After you consent, you may request a free paper copy of any disclosures or notices that you have agreed to receive electronically by writing or calling us at Credit Union of Texas, P.O. Box 515169, Dallas, TX 75251-5169, (972) 263-9497 or (800) 314-3828. You may withdraw your consent to receive future electronic disclosures any time by e-mailing us at feedback@cuoftexas.org. Your withdrawal will become effective after we have received it and have had a reasonable opportunity to act on it.

3. If your e-mail address with us changes, you may e-mail us your updated information at feedback@cuoftexas.org. If any of your other contact information on file with us changes, you must provide your updated information to us at Credit Union of Texas, P.O. Box 515169, Dallas, TX 75251-5169. Please include your name and account number when contacting us.

4. To access the disclosures you have agreed to receive electronically, you will need a computer with a working Internet connection, a minimum web browser version of Internet Explorer 3.0 or Netscape Navigator 3.0, and Adobe® Acrobat® Reader®. If you don't have Adobe Acrobat Reader, you may download it here. In order to retain the disclosures, you will need a printer or at least 136 KB of available space on your hard drive.

If you click "I Do Not Agree", please contact our Lending Services department at (972) 263-9497 or (800) 314-3828 to apply for your loan. An application and disclosures will be mailed to you.
